with their aftercare and discharge plan.

team will respond to your question. Please include details regarding the
steps you have taken, what is working, and concerns to which you would
like us to respond.

of your information will be shared with a third party. The primary purpose
of this system is to encourage communication between former patients, family
and parents with our staff. After a gathering period of "question and
answer" submissions and responses, we plan on displaying ANONYMOUS questions
with answers on this website for the general public. Once again, no part of
your personal information will be revealed or shared with a third party. In
the event that it is necessary to rephrase portions of your submission prior
